Passport Service
U.S. passportsPatrons can apply for a U.S. passport and/or passport card at three Birmingham Public Library locations: Central, Avondale, and Springville Road.These library locations are officially designated by the U.S. Department of State as passport application acceptance facilities.

 

 

How To Apply

Please note that, there are special requirements for minors under age 16, minors age 16 and 17, and no-fee passports when submitting Form DS-11.

STEP 1: Complete and Print Form and Call for an Appointment

Enter the information online and print Form DS-11 OR
Print a blank Form DS-11 and complete by hand OR
Complete the DS-11 form at the library.

Note: Do not sign the DS-11 form until the passport agent instructs you to do so.

STEP 2: Submit Completed Form + Additional Documents

(For applicants 17 years old or younger, you must provide a birth certificate in addition to one of the following)

  1. Proof of U.S. Citizenship (one of the following)

    U.S. birth certificate
    Previous U.S. passport
    Naturalization Certificate
    Citizen Certificate
    Certificate of a U.S. Birth Abroad


  2. Proof of Identity (one of the following; originals only - no copies)

    Valid driver's license (not temporary or learner permit)
    Previous U.S. passport book or passport card
    Certificate of Naturalization with identifiable photo attached
    Certificate of Citizenship with identifiable photo attached
    Government employee identification card (federal, state, county, municipal)
    Official U.S. military or military dependent identification card
    Current (valid) foreign passport

  3. Photograph*

Applicants must present one passport photograph taken within the past 6 months. Photos must be 2x2 inches in size (no exceptions). It should be front view with a white background. The applicant must be the only person in the photo.

*At the Central Library and the Springville Road Regional Branch Library, passport photos may be purchased for a $15.00 fee.


STEP 3: Pay the Applicable Fees

TWO payments are required: One made payable to the U.S. Department of State (passport application fee) and one made payable to Birmingham Public Library (execution fee).

The passport application fee and any additional services fees (e.g. expedited service) may only be paid for with a CHECK (personal, certified, cashier's or traveler's) or MONEY ORDER payable to the U.S. Department of State.

The execution fee may be paid for with a check, money order, cash, or credit card payable to the Birmingham Public Library.

To submit a passport application at the Avondale Regional Branch Library and the Springville Road Regional Branch Library, you will need to make an appointment.  Walk in applicants are accepted Monday through Friday at the Central Library ONLY.
